Documents Required For WB Health DV
The candidates must bring under noted original documents for WB Health Laboratory Technician, Medical Officer, Counselor Under Polyclinics/ Specialist Service, Staff Nurse, Community DV, along with one set of self-attested photocopies of the same, for verification and submission which was submitted during the online application.
1. Online application print-out.
2. Proof of Address issued by any competent authority.
3. Age-proof certificate (Madhyamik or equivalent examination Admit / Certificate)
4. All the Essential marks sheets (for all years/ semesters) and course completion certificates and documents in support of desirable qualification and computer course completion certificate as applicable.
5. All the post qualification experience certificates issued and stamped by the appropriate authority starting from older to lasted. (Experience certificates must consist of the Name of the post, Employer’s Name, Employee’s Name, Date of joining, and Date of Leaving /current date in case still continuing otherwise his/her experience will be treated as invalid. No offer of appointment /engagement will be treated as experience).
6. Documents relating to Guardian’s identity. (Guardian as mentioned in the online application).
7. Original Demand Draft in favor of District Health & Family Welfare Samiti, Dakshin Dinajpur payable at Balurghat. Without submission of DD candidature will be canceled.
8. And any other requisite documents.
